Payment for lessons can be mailed in along with these forms.

One official form must be completed for each participant. The forms may be photocopied or you may obtain extra forms and brochures from any pool location, aquaknowaquatics.org or by calling (512) 377-AQUA (2782).

• EACH STUDENT IS ENCOURAGED TO HAVE DOCTOR'S APPROVAL TO PARTICIPATE IN EACH CLASS.
• Participation agreement must be signed. Incomplete forms will be returned and your initial postmark date will be canceled
• Class fees vary depending on class and location. Please see site location for pricing.
• Only full payment will reserve a space in a course
• Mail in Deadline is 7 days before the course begins (post marked)
• There is a $5.00 late registration fee after the walk in/ mail in deadline
• Discounts are available. A total discount of $5 off the total amount may be applied for one of the following:
• Multiple enrollment. An individual is enrolled in more than one course.
• Multiple sibling enrollment. Two or more siblings are enrolled in the program.
• Adult Student. Please provide proof of enrollment.

You will be notified only if your first choice is not available. Please indicate a second choice on the registration form. You will be placed on a waiting list in case there is a later opening.

Refunds: Due to enrollment demands in our classes, if you withdraw PRIOR to the FIRST DAY of class you will receive a refund minus a $10.00 process and handling fee. Allow 6-8 weeks for refund checks.

THERE ARE NO REFUNDS IF YOU WITHDRAW AFTER THE FIRST DAY OF CLASS.
